KB-R7943 is a potent, selective inhibitor of the reversed Na⁺/Ca²⁺ exchanger (IC₅₀ = 0.7–2.4 µM in various cells). It also inhibits the mitochondrial Ca²⁺ uniporter (Kᵢ = 5.5 µM), mitochondrial complex I (IC₅₀ = 11.4 µM), and blocks NMDA receptors (IC₅₀ = 13.4 µM).
