Centrally mediated abdominal syndrome (CAPS) is one of the so-called disabilities of the gut-brain interaction (DGBI), also known as functional gastrointestinal disorders (FGID). It is characterized by persistent or almost persistent abdominal pain or repeated episodes of pain for more than six months. CAPS is less common than other functional gastrointestinal diseases such as irritable bowel syndrome (IBS) and functional dyspepsia.

The pathogenesis of CAPS might be linked to gut-brain axis abnormalities. Abdominal pain, dyspepsia, and constipation could be possible manifestations of the functional impairment of the gut-brain axis. CAPS is believed to result from central sensory processing control rather than peripheral stimulation. Once central sensitization has developed, abdominal pain can be present independent of peripheral stimulation. Some CAPS researchers reasoned that the overriding factor initiating CAPS was dysfunction in the descending pain control system, while the mental disorder appeared to be a predisposing factor.
