Description

2-Fluoro-2-deoxy-d-glucose [FDG] is a glucose analog that inhibits cellular glycosylation. It is the natural isotope form of 18F-FDG and can be taken up by cells but does not undergo metabolic glycolysis. FDG uptake in malignant tumors is closely related to the expression of glucose transporter (GLUT). FDG is also a substrate for hexokinase isoenzymes and an inhibitor of cellular glycosylation. FDG is taken up by high glucose-consuming cells such as the brain, brown fat cells, kidneys, and cancer cells. Once glucose is taken up, phosphorylation in these cells prevents glucose from being released from the cell again. The 2-hydroxyl group (-OH) in normal glucose is required for further glycolysis (metabolism by splitting glucose), but FDG lacks this 2-hydroxyl group and cannot be further metabolized in the cell. FDG is used as a contrast agent in positron emission tomography (PET) and is currently the most widely used PET marker. FDG-PET can be used for the diagnosis, staging and treatment monitoring of cancer, especially in Hodgkin's disease, non-Hodgkin's lymphoma, colorectal cancer, breast cancer, melanoma and lung cancer. FDG also has anticancer and chemosensitizing activities.
